TERMS OF AGREEMENT

• $90 annual membership enrollment fee

• Service payments occur on the 1st of each month: $90

• Memberships process on an automated, subscription basis on the 1st of each month.

• There is a three month minimum commitment; after three months your membership continues at

• Automatically on a month-to-month basis

• If you need to end your membership, please give us 30-days written notice, via email, prior to billing, to avoid charges to your card.

• You may roll over up to three massages. Any rolled over massages expire after three months from the date of purchase.

• Need to reschedule? Please note that our 48-hour cancellation policy applies to all members and non-members.
